29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. a. The Roof - On or about April 22, 2024, employees were working on a low slope roof performing patching work, the employer was using a warning line system with an area monitor for fall protection. The warning lines however were not arranged and weighted to prevent moving resulting in the sag reducing the height of the warning line to less than 34 inches from the walking working surface. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. The Roof - On or about April 22, 2024, employees utilized a Werner 24-foot fiberglass extension ladder that was not positioned to extend at least 3 feet above the landing where employees were accessing and egressing from the roof. Employees were exposed to a fall hazard of approximately 15 feet. Corrected During Inspection

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines: a. The Roof - On or about April 22, 2024, employees were working on a low slope roof performing patching work, the employer was using a warning line system with an area monitor for fall protection. The warning lines however were not arranged to establish an access point for personnel entering and exiting the roof. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
