29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On or about April 22, 2024, and at times thereto, where employees performed framing activities from an unapproved job made work platform used to elevate the employees with a JCB telehandler forklift.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about April 22, 2024, and at times thereto, employees installing framing material onto a commercial building were working inside an elevated platform not secured to the forklift or forks to prevent the platform from falling off the forklift exposing employees to falls of approximately 19-feet to the ground below.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about April 22, 2024, and at times thereto, employees performed framing activities from an elevated job made work platform and fall protection was not provided.
