Employees used the attachment to lift bundles of granite weigh over 8000 pounds.

Warehouse Areas: On or about April 22, 2024, and times prior, the employer exposed employees to struck-by hazards, in that powered industrial truck (PIT) training did not include practical training for employees operating the "Linde" diesel forklift model H80D/900 (serial number H2X396F51077/2015) "Komatsu" LP forklift, model # FG30SHT-12 (serial number Serial # 558607A) and Yale LP forklift, model GLC155VXNEJE112, (serial #F879V02022P) who use the forklifts to move heavy slabs of granite.

Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer. a. Warehouse Areas: On or about April 22, 2024, and times prior, the employer exposed employees to struck-by hazards, in that the employer used counterweights on a Yale LP forklift, model GLC155VXNEJE112, (serial #F879V02022P) where the counter-weights were not approved by the forklift manufacturer as the forklift was used by employees to move heavy equipment.

Slings were used by employees to lift slabs of Granite weighing at least 3000lbs. b) Warehouse floor (Keeper black strap 3333lbs) On or about April 22, 2024, and times prior, the employer exposed employees to struck-by hazards, in that damaged/defective lifting straps were not replaced or removed from service. Slings were used by employees to secure slabs of Granite while unloading.
