To fully access the cyclone door, the employee stood on the mid-rail of the platform guard-rail, exposing the employee to a fall hazard:  (a) On or about April 18, 2024, an employee opened the cyclone door of the Mogul - 1: Tomra process to clear a blockage of product.  The access door was not guarded to prevent operation of cyclone during clearing of a blockage or other minor adjustments to the cyclone operations, exposing the employee(s) to a caught-in/pinch hazard and amputation hazard.
