29 CFR 1926.1053(b)(1): Ladder extension was as such that ladder deflection under a load, did by itself, cause the ladder to slip off its support: On or about April 23, 2024, and at times prior thereto, employees engaged in roofing activities were using portable extension ladders with the side rails that did not extend at least three (3) feet above the upper landing surface to access the roof. The employees were exposed to fall hazards of approximately six (6) feet or more above the lower level during the following instances (a-b): a) One (1) ladder at the back of the home on the left side. b) One (1) ladder at the back of the home on the right side.
