29 CFR  1910.120(q)(1): The employer did not develop and implement an emergency response plan to handle anticipated emergencies prior to commencement of emergency response operations:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR DEPARTMENTS AND THE NUMBER OF EMPLOYEES EXPOSED) (DESCRIBE CONDITIONS)  a) At the establishment located at 1997 American Blvd, De Pere, Wisconsin; the employer failed to implement the emergency response plan by not following the plan that was developed to evacuate the facility during a chemical release and failed to provide trained personnel, such as but not limited to the employee acting as the incident commander, exposing employees to chemical hazards. On or about April 17, 2023, while manufacturing BCL AHS LO PH 1100_1GL_4CT (Wheel Cleaner), an exothermic reaction occurred after adding sulfuric acid to Tank M11 which resulted in an overflow.
