On or about April 22 2024, near the intersection of Bridgeland Creek Parkway and Grand Parkway in Cypress, TX, employees working in an excavation were exposed to struck-by hazards when the spoil pile was less than 2 feet from the edge.

29 CFR 1926.652(b):The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): On or about April 22 2024, near the intersection of Bridgeland Creek Parkway and Grand Parkway in Cypress, TX, employees were exposed to crushed-by hazards when tying in an electrical line in an excavation that exceeded a depth of 5 feet with a sloping and benching system that did not meet the allowable configurations.
