29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  On or about April 22 2024, near the intersection of Bridgeland Creek Parkway and Grand Parkway in Cypress, TX, employees working in an excavation were exposed to struck-by hazards when the spoil pile was less than 2 feet from the edge.

29 CFR  1926.652(b):The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  On or about April 22 2024, near the intersection of Bridgeland Creek Parkway and Grand Parkway in Cypress, TX, employees were exposed to crushed-by hazards when tying in an electrical line in an excavation that exceeded a depth of 5 feet with a sloping and benching system that did not meet the allowable configurations.
