29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about April 24, 2024, the employer failed to ensure that employees used appropriate eye or face protection when spraying corrosive chemicals, such as but not limited to, Chroma/Strip Plus iSC, in the screen cleaning booths. No goggles were provided to employees.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about April 24, 2024, the employer failed to provide effective information and training on hazardous chemicals in the work area when no training had been provided to employees on the chemicals that they work with. Employees are exposed to various chemicals including inks, adhesives, and cleaning chemicals throughout the production area.

29 CFR  1910.38(f)(1): The employer did not review the emergency action plan with each employee covered by the plan when the employee was assigned initially to a job:  On or about April 24, 2024, the employer did not review the emergency action plan with each employee at the time of initial assignment to a job. No training on the plan had been provided to any employees since 2018.
