29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.   a. Nitro Roofing and Construction LLC located at 4730 E Dublin Granville Rd, Westerville, OH 43081: On or about April 24, 2024,  the controlling employer did not ensure site safety inspections were conducted by a competent person when sub-contractor employees were engaged in construction work at heights that exposed them to falls over 30 feet.

29 CFR  1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.   a. Nitro Roofing and Construction LLC located at 4730 E Dublin Granville Rd, Westerville, OH 43081: On or about April 24, 2024,  the employer did not ensure employees engaged in roofing activities at heights over 6 feet were protected from falls. The following condition exposed the employee to falls between 12 and over 30 feet.  Nitro Roofing and Construction  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1636344, citation number 1, item number 1  and was affirmed as a final order on February 13, 2023, with respect to a workplace located at 5059 OH 29, Celina, OH 45822.

1926.1053(b)(9):  The area around the top and bottom of ladders shall be kept clear:  a. Nitro Roofing and Construction LLC located at 4730 E Dublin Granville Rd, Westerville, OH 43081: On or about April 24, 2024, the employer did not ensure that portable ladders used to gain access to the roof extended at least three feet beyond the upper landing that they were using for access and/or were secured at the top with a grasping device. One extension ladder was used to gain access to roof and was was positioned approximately 12 inches below landing surface and was not secured in any way which exposed employees to falls over 18 feet.  Nitro Roofing and Construction  was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1636344, citation number 1, item number 2  and was affirmed as a final order on February 13, 2023, with respect to a workplace located at 5059 OH 29, Celina, OH 45822.
