29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On April 24, 2024, an extension ladder used by employees to gain access to the working surface did not have its side rails extended 3 feet above the upper landing being accessed.  Etruscan Exteriors Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 1528397, citation 1, item 3, and was affirmed final order on June 15, 2021, with respect to a workplace located at 1315 Richmond Ln, Wilmette, IL 60091.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On or about April 24, 2024, at 1241 Deerfield  Rd, Deerfield, IL, the employees were exposed to fall hazards of up to approximately 8.5 feet ground to eave while performing residential roofing work.  No abatement documentation or certification is required for this item.
