29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about April 15, 2024, the employer did not ensure that the personal fall arrest systems were rigged to limited the fall distance to 6 feet, nor prevented contact with the lower level, in that on April 15, 2024, an employee conducting framing operations on the third story of Unit No. 45 utilized a harness, lifeline and rope grab where the rope gab was positioned at the end of the lifeline. This condition resulted in an employee falling approximately 30 ft. to ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about April 15, 2024, and at times prior, employees were exposed to falls of approximately 30 feet to the ground below while conducting framing activities on a new multi-family residential structure. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.
