29 CFR 1926.451(b)(5)(ii):  The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:  (a) South side of residential home, west scaffold plank; On or about April 22, 2024, and dates prior, the right end of a Werner brand (model 2316) 16-foot-long scaffold plank extended/cantilevered 5-feet, 7-inches beyond its support bracket before it was connected to and/or supported by the middle plank with a crowbar installed between the hand grips at the end of each plank to make one continuous working platform.  (b) South side of residential home, middle scaffold plank; On or about April 22, 2024, and dates prior, the left end of a Werner brand (model 2316) 16-foot-long scaffold plank supported the west scaffold plank with a crowbar between its end handles while also being extended/cantilevered 22-inches beyond the support bracket.  The right side of the plank extended 14-feet past the support bracket and was fully supported by the east scaffold plank with a crowbar between its end handles.
