29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  Location: a) Supported scaffold on the exterior of the jobsite:  On or about April 23, 2024, employees were exposed to fall because a supported scaffold was not fully planked, was not provided with fall protection for the employees, and was not provided with safe means of access for the employees. The employer did not have a competent person to provide frequent and regular inspection at the jobsite.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Location: a) Supported scaffold on the exterior of the jobsite:  On or about April 23, 2024, employees were working on the façade of the building while standing on the first-tier and second tier platforms of a four-tier-high supported scaffold that was not fully planked or decked between front upright and the guardrail supports, and were exposed to falling up to approximately 12 feet to the lower level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold frames were used as a means of access.  Location: a) Supported scaffold on ground level of the jobsite:  On or about April 23, 2024, employees were working on the façade of the building while standing on the first-tier and second tier platforms of a four-tier-high supported scaffold. There was no safe means of access provided. The employees had to climb the scaffold frame and the crossbraces to get to the platform. The employees were exposed to falling up to approximately 12 feet to the lower level.

Location: a) Supported scaffold on the exterior of the jobsite:  On or about April 23, 2024, employees were working on the façade of the building while standing on the first-tier and second tier platforms of a four-tier-high supported scaffold. The employees were not protected by personal fall arrest systems or guardrail systems or other fall protection systems. The employees were exposed to falling up to approximately 12 feet to the lower level.
