On or about April 24, 2024, at the above addressed jobsite, an employee required to operate a JLG Skytrak Model 6042 Powered Industrial Vehicle (PIV) had not been trained and evaluated on the safe operation of the powered industrial truck, thereby exposing employees to vehicle tip-over, struck-by, and crushed-by hazards.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a. On or about April 24, 2024, at the above addressed jobsite, an employee performing carpentry operations from a platform raised, approximately twenty-five (25) feet above ground, by the forks of a JLG Skytrak Model 6042 Powered Industrial Vehicle (PIV), did not have the platform attached to the forks, thereby exposing the employee to fall hazards.
