29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)	Emanuel Schwartz DBA Schwartz Brothers Construction. On or about March 12, 2024 at the address of 204 South Linden St. Jackson Center OH 45334. The employer did not ensure that aerial lift operator donned and secured their personal fall arrest systems before operating JLG aerial lift (serial# 0300154553 Model#460SJ). Employees utilizing aerial lift to performed framing work activities (installing prefab wooden trusses) were exposed to fall hazards of approximately 18 feet to the ground below.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a)	Emanuel Schwartz DBA Schwartz Brothers Construction. On or about March 12, 2024 at the address of 204 South Linden St. Jackson Center OH 45334. The employer did not ensure that each employee working from JLG aerial lifts (serial# 0300154553 Model#460SJ) were trained in the recognition of hazards associated with those types of mobile scaffold. Employees performing framing work activities (installing prefab wooden trusses) on a commercial wood frame structure were exposed to fall hazards of approximately 18 feet to the ground below.

On or about March 12, 2024 at the address of 204 South Linden St. Jackson Center OH 45334: The employer did not ensure employees were protected from falls while performing framing work activities (installing prefab wooden trusses) atop a single-story commercial structure. Employees walking top plate of stub walls were exposed to fall hazards of 18 feet to the ground below.
