29 CFR 1910.147(f)(1)(v): All systems were not de-energized and energy control measures reapplied in accordance with paragraph (d) of this section to continue the servicing and/or maintenance: At the warehouse, on or about April 29, 2024, employee(s) were exposed to crushing and/or caught-in between hazards. The employee(s) engaged in troubleshooting on the 3021-trailer loader machine were exposed to crushing/caught in-between hazards in that alternative protection measures were not implemented to protect the employee(s) resulting in amputation and/or lacerations.
