29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:   a. On or about April 29, 2024, at the worksite, employees were exposed to 8 foot 8 inch fall hazards while scraping off shingles and underlayment on a 5/12 pitched roof, in that, fall protection was not used.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about April  29, 2024, at the worksite, the employer did not prepare and maintain a written fall protection training program certification for employees exposed to 8 foot 8 inch fall hazards while performing roofing activities.
