29 CFR  1904.29(a): The employer did not use an OSHA 301 Form or equivalent.  a.) FFC, LLC dba Frecon Farms, 501 South Reading Avenue, Boyertown, PA  19512, on or about January 14, 2024 - the employer's incident report that was used to explain an incident that caused a right index and right middle fingertip amputation with a Reiser Vemag Cookie Machine was not documented on a standard OSHA Form 301, or an equivalent form to the OSHA Form 301, in that it did not include: - the job title and phone number of the person who completed the report; - the injured employee address, date of birth, and date hired;  - the time that employee began work on the day of the incident; - the name of the physician or other health care professional providing care: and  - the address of the hospital in which the employee was taken for treatment.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.29(b)(3):  Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  a) FFC, LLC dba Frecon Farms, 501 South Reading Avenue, Boyertown, PA 19512, on or about January 14, 2024 - the employer did not enter a right index and right middle fingertip amputation that occurred with the Reiser Vemag Cookie Machine on the OSHA 300 Log for calendar year 2024 within seven calendar days of receiving information that a recordable injury had occurred.  Abatement certification and documentation required within 10 days after abatement date.
