Location - On the ground level  On or about 4/29/2024, employees on the ground level were not protected from falling debris/objects hazards while working under other elevated employees.

29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   Location - At the roof ridgeline  On or about 4/30/2024, employees on a residential roof were not protected from falls. The fall protection anchor point (roof bracket) was not installed to prevent injury and/or fall.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   Location - Roof  On or about 4/30/2024, employees were exposed to injury and a fall greater than 22 feet in height to ground level from a residential roof while working where the fall protection system being utilized (leg/waist harnesses) were inadequate to prevent injury and/or fall.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location - Roof  On or about 4/30/2024, employees on a residential roof were not protected from falling to a lower level while working at heights greater than 6 feet.  The Mass Development Group Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1634230, citation number 1, item number 1 and was affirmed as a final order on 03/08/2023, with respect to a workplace located at 204 Amesbury Road, Haverhill, MA 01830.
