29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 30, 2024, on the east side of the building facing South Wood Drive, an employee was working from an upper level that was more than six (6) feet to the concrete ground and was not in use of fall protection.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a ridge. On or about April 30, 2024, on the east side of the building facing South Wood Drive, an employee used a portable ladder to gain access to an upper landing surface and due to the length of the ladder the side rails did not extend at least three (3) feet above the landing.
