Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 30, 2024 the employer Marlin Beachy Jr. DBA JR's Roofing at jobsite located at 2422 Torino Dr. Port Clinton OH 43452, did not have a fall protection training program in place to enable each employee to recognize the hazards of falling, and procedures to minimize these hazards. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  On or about April 30, 2024 the employer Marlin Beachy Jr. DBA JR's Roofing at jobsite located at 2422 Torino Dr. Port Clinton OH 43452, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Two employees were exposed to falls from approximately 19 feet or greater from the top of the second (roof) level to the decking below while using a Werner brand fiberglass ladder to access the second (roof) level to perform residential framing activities.

Two employees were exposed to falls from approximately 19 feet or greater from the top of the second (roof) level to the ground below while using a Werner brand fiberglass extension ladder to access the second (roof) level to perform residential roofing activities on a single family home.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about April 30, 2024 the employer Marlin Beachy Jr. DBA JR's Roofing at jobsite located at 2422 Torino Dr. Port Clinton OH 43452, did not ensure that each affected employee used appropriate eye or face protection when performing residential roofing and activities on a new build residential structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns. Two employees were exposed to eye struck by hazards while installing shingles while using Bostitch brand roofing pneumatic nail guns.  Marlin Beachy Jr. DBA JR's Roofing was previously cited for a violation of this Occupational Safety and Health Standard CFR 1926.102(a)(1) which was contained in OSHA inspection number 1611040, citation number one, item number one and was affirmed as a final order on October 19, 2022 with respect to a workplace located at 5061 Shepards Glen, Willoughby, OH 44094.  Marlin Beachy Jr. DBA JR's Roofing was previously cited for a violation of this Occupational Safety and Health Standard CFR 1926.102(a)(1) which was contained in OSHA inspection number 1397848, citation number one, item number one and was affirmed as a final order on June 12, 2022 with respect to a workplace located at 6021 Valley Quail, SEVILLE, OH 44273.

Employees were exposed to falls of approximately 19 feet (from the eaves of the third level roof peak) or greater to the ground below. The roof was a 10:12 pitch.  Marlin Beachy Jr. DBA JR's Roofing was previously cited for a violation of this Occupational Safety and Health Standard CFR 1926.501(b)(13) which was contained in OSHA inspection number 1611040, citation number one, item number two and was affirmed as a final order on October 19, 2022 with respect to a workplace located at 5061 Shepards Glen, Willoughby, OH 44094.
