29 CFR 1910.147(e)(1):Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact. (a)Millwood, Inc. 435 Independence Ave, Mechanicsburg, PA. Millwood Infeed Line: On or about April 12, 2024, a maintenance employee, making repairs to the Millwood infeed line, removed the lockout device and restored energy to the machine while remaining within the point of operation collecting tools. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a)Millwood, Inc. 435 Independence Ave, Mechanicsburg, PA. Millwood Infeed Line: On or about April 12, 2024, authorized employees removed lockout device and put the feed line back into use and did not ensure that all employees had been safely positioned away from the work area prior to resuming normal operations. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
