29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Commercial building located at 4301 Mead Road, Macon, GA 31206, Roof: On or about 4/30/2024, the employer exposed employees to fall hazards at heights of approximately 24-feet 6-inches, in that no fall protection system was provided to all employees while engaged in roof activities on a low sloped roof.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a) Commercial building located at 4301 Mead Road, Macon, GA 31206, Ground level: On or about 4/30/2024, the employer exposed employees to struck-by hazards, in that the employer did not ensure that the forklift operator on site had been trained and certified to operate the Genie forklift truck used to elevate equipment and materials to the roof.
