Lincoln Ave., Loveland, CO 80537: On and about April 29, 2024, the employer did not ensure that employees, who conducted work from the fabricated frame scaffold, were provided a ladder for safe access and egress to the scaffold. This condition exposed employees to the hazard of falling from elevation at a distance of approximately 13 feet.

This condition exposed the employee to a fall hazard of approximately 10.6 feet. (b) 1039 S. Lincoln Ave., Loveland, CO 80537: On or about April 29, 2012, the employer did not ensure that employees were protected from injury, in that employees were using a wood plank to transition between steel awnings a distance of approximately six feet horizontally. This condition exposed the employee to a fall hazard of approximately 10.9 feet.
