Instance A) On or about May 1st,  2024, at 201 N Summit Street Toledo, Ohio 43604 DMG Contractors Inc., failed to implement engineering controls, work practices and respiratory protection specified for the task in Table 1. , Employees were exposed to respiratory hazards including respirable crystalline silica while using a Stihl brand handheld power saw with no dust collection vacuum system and no water delivery system as specified in Table 1.
