29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) 1009 Birdwhistle Ln, Ft Collins, CO 80524: On April 30, 2024, and at times prior, employees were exposed to falls of approximately 12 feet to the ground below while conducting framing activities on a new residential home. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: a) 347 Pratt St, Longmont, CO 80501: On April 22, 2024, and at times prior, employees were exposed to falls of approximately 12 to the ground below while accessing/egressing upper levels of the existing residential home. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.
