29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) B & B Molded Products on or about April 23, 2024 at the location of 1250 Ottawa Avenue Defiance, OH 43512: The employer did not ensure established lockout procedures were followed in sequence to protect employees from potentially hazardous energy when performing service and maintenance task. Due to this condition, operators tasked with clearing yarn jams from Toilet Mop Line 2 were exposed to injuries associated with being caught in rotating parts and nip points.
