The competent person did not perform an inspection of the scaffold before each work shift.

29 CFR 1926.503(a)(2)(iii):The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) Worksite - On or about May 2, 2024, employees are utilizing personal fall arrest systems to perform work tasks on scaffolding. The employer did not ensure employees follow the manufacturer's instructions for use and storage. A fall arrest harness was stored on the ground, in direct sunlight near heavy equipment traffic flow.
