29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite located at 4324 Lancaster Ave. Philadelphia PA 19104 - On or about May 2, 2024, the employer did not ensure employees installing concrete foundation walls for a mixed use building, were protected from fall hazards by the use of a fall protection system. Employees were working from the top of the formwork, approximately 9 feet above ground level without the use of fall protection.
