29 CFR  1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Employee(s) were exposed to fall hazards when accessing and working from the storage area above the office. This was documented on or about May 2, 2024 at 4524 South 99th Street, Omaha, Nebraska. Employee(s) were exposed to a fall hazard due to a section of the guard rail system not being installed along the edge of the storage area above the office. The distance from the edge of the office area to the ground was measured at 9 feet with a trench rod.

This was documented on or about May 2, 2024 at 4524 South 88th Street, Omaha, Nebraska. Employee(s) were exposed to chemical hazards due to the lack of an eyewash station. The employer utilizes several corrosive chemicals including but not limited to; Grime Fighter CW, Liquid Brush, and Wall-Klean, and do not have access to an eyewash station.

This was documented on or about May 2, 2024 at 4524 South 88th Street, Omaha, Nebraska. Employee(s) were exposed to electrical hazards due to a fuse box being inaccessible. The fuse box was surrounded by materials which made the fuse box inaccessible.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Employee(s) were exposed to electrical hazards when using or working near a damaged light switch and a damaged outlet. This was documented on or about May 2, 2024, at 4524 South 88th Street, Omaha, Nebraska. Employee(s) were exposed to electrical hazards due to the front plate of a 2-gang switch being broken, exposing live parts, and a damaged outlet box which has had its front plate unseated from the backplate, exposing live parts.   A) Damaged light switch in the chemical side of the warehouse near the door to the office. B) Damaged circuit box in the chemical side of the warehouse near the door to the office.
