29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about May 2, 2024, at the U.S. Customs and Border Protection, Air and Marine Operations, Ceiba: inside the break room - there was a fire extinguisher, model 10 HI SA80 ABC, serial number E63512401, missing a breakable seal and no documentation on the card of a monthly inspection, exposing employees to fire hazards. b) On or about May 2, 2024, at the U.S. Customs and Border Protection, Air and Marine Operations, Ceiba: on the Southeast corner of the office - there was a fire extinguisher, model 10HI SA80 ABC, Serial Number E63512633, with low pressure and no documentation on the card of a monthly inspection, exposing employees to fire hazards. c) On or about May 2, 2024, at the U.S. Customs and Border Protection, Air and Marine Operations, Ceiba: inside the Weapons Ammunition Room #201 - there was a fire extinguisher, model 10HI SA80 ABC, Serial Number E63512387, obstructed by boxes, exposing employees to fire hazards. d) On or about May 2, 2024, at the U.S. Customs and Border Protection, Air and Marine Operations, Ceiba: inside the Weapons Ammunition Room #201, there was a Fire Extinguisher, Class D, Model B570, Serial Number AX-156788, obstructed by a case, exposing employees to fire hazards. NOTE: Certification and documentation of abatement must be submitted for all instances of this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) On or about May 2, 2024, at the U.S. Customs and Border Protection, Air and Marine Operations, Ceiba: inside the Blue Tool Shed Connex - a Dayton 6" bench grinder, serial number 18010015, base plate was adjusted 4/8 of an inch instead of 1/8 inch from the wheel, which can cause the work piece from being jammed between the wheel and the rest, resulting in wheel breakage producing flying particles or pull the employees' hands into the wheel, exposing them to eye and hand injuries. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
