29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) Fabrication Shop: On March 4, 2024 an employee was struck on the right foot by a load that slipped from the crane rigging during transit, which ultimately lead to a partial amputation of the right pinky toe. The work-related amputation was not reported to OSHA by the company within 24 hours.

29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: (a) Fabrication Shop: On or about May 2, 2024 and times prior, employees would utilize slings in conjunction with an overhead crane to move material throughout the shop. The slings were not inspected each day prior to being used.
