29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Worksite was located at 102 2nd Avenue, Belmar, NJ 07719:  A worker was using a makeshift scaffold supported by a JLG SkyTrak 8042 Telehandler to sheath the chimney of an approximately four story multi-family residential building  with no fall protection in place. The makeshift scaffold was only secured by two nylon straps that stretched  from fork to fork. The makeshift scaffold was not secured to the mast. The worker was not properly trained in scaffolding to recognize these hazards.  Violation occurred on or about May 2, 2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. The SkyTrak Legacy Series Owner's / Operator's Manual includes within its Operation section, a Pre-Operation Checklist that includes seven items to be checked before operating the vehicle.  Violation was observed on or about May 2, 2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
