29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: (a) Grinding area, Wolfking line 5; On or about April 29, 2024, and dates prior, electrical energy was not isolated before jam clearing operations were conducted on the Wolfking line 5 meat separator machine (TPM 800). A Wolfking line 5 operator's hand was surgically amputated after being pulled into a TPM 800 meat separator that unexpectedly started running while they were clearing hung-up/jammed material from inside the hopper.
