The employer did not get the forklift manufacturers' prior written approval to use the attachments, which could result in hazards such as overloading the forklift or dropping a personnel platform and its occupants, on or about May 8, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph [General Industry reference 1910.27(a)]: (a) 98 East Main Street, Leola, Pennsylvania - To reach elevated work, employees used an in-house made personnel platform (with orange guardrails) elevated on the forks of any one of the following powered industrial trucks: Nissan forklift (model BGF03A45V, chassis number BGF03-920953), UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21014), or UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21246). The guardrail on this personnel platform did not meet the requirements of a guardrail system in that the side where employees entered the platform had a chain at the height of a top rail but had no mid-rail or other intermediate member, exposing employees to the hazard of falling to a lower level, on or about April 1, 2024. (b) 98 East Main Street, Leola, Pennsylvania - To reach elevated work, employees used a large in-house made personnel platform (with a rusted patina) on the forks of any one of the following powered industrial trucks: Nissan forklift (model BGF03A45V, chassis number BGF03-920953), UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21014), or UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21246). The guardrail on this personnel platform did not meet the requirements of a guardrail system in that it had only a top rail and no mid-rail or other intermediate member and had no guardrail at the opening for the integrated ladder. In addition, the top rail had become completely detached from an upright on one end of the platform, exposing employees to the hazard of falling to a lower level, on or about May 8, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

The station used to bend clips and to stamp grate guide pins had no guarding, and the station used to punch holes in clips had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the points of operation, on or about April 1, 2024. (b) Shear and Press Area - Employees hand-fed metal parts into and used a foot pedal to actuate the Cincinnati mechanical shear, and the point of operation had no guarding, exposing employees to getting struck-by the shear blade, on or about May 6, 2024. (c) Shear and Press Area - Employees hand-fed metal parts into and used a foot pedal to actuate the Pacific hydraulic shear, model 600 G12, serial number S11176. The fixed guard at the right side of the machine had become detached, which would allow employees' fingers and hands to reach into the shear blade at the right side of the machine, on or about May 6, 2024. (d) Shear and Press Area - Employees hand-fed metal parts into and used a foot pedal to actuate the Pacific hydraulic press brake, and the point of operation had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about May 6, 2024. (e) Shear and Press Area - Employees hand-fed metal parts into and used a foot pedal to actuate the Cincinnati mechanical press brake, series 12, serial number 37055, equipped with a punch, and the punch had no guarding, exposing employees to the hazards of getting hands and fingers struck-by or caught-in the point of operation, on or about May 6, 2024.
