29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about April 24, 2024, at the Un-Coiler, Pipe Forming, Automatic Welding and Bundling area; employees were working in proximity to rotating equipment and moving parts without guarding at the point operation while conducting pipe fabrication activities.

On or about April 24, 2024, at the Un-Coiler area; employees were welding without the use of protective screens/barriers to shield other personnel in the area from noise, fumes, and UV light exposures.

On or about April 24, 2024, at the pipe forming equipment and conveyor/packing area; an employee was fabricating and bundling piping in proximity to high voltage power cables in standing water while the employee stood on metal grating operating the equipment.
