On or about 5/2/2024, during the installation of roofing material at a job site located at 1221 Wales Rd NE, Massillon Ohio 44646; the employer did not ensure employees working on the roof of a residential single-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 5/2/2024, during the installation of roofing material at a job site located at 1221 Wales Rd NE, Massillon Ohio 44646; employees were exposed to a fall hazard of approximately 10 feet to the ground while working on a roof of a single-family home installing roofing material without the use of fall protection and or a fall protection system. Cochran Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1476788, citation number 01, item number 02 and was affirmed as a final order on (6/26/2020), with respect to a workplace located at 6381 Blossom Wood LN, Canton Ohio 44721.
