(A) Pathology Lab, on or about 7 May 2024 and times prior thereto: Employees are exposed to 10 % formaldehyde (Formalin) while preforming tissue sample preservation procedure of a workplace covered by 29 CFR 1910.1048 were not monitored to determine their exposure to formaldehyde (formalin),resulting in potential chronic and irreversible illnesses.

The employer shall institute engineering and work practice controls to reduce and maintain employee exposures to formaldehyde at or below the TWA and the STEL. (a) Pathology Lab, on or about 7 May 2024 and times prior thereto: Employees were exposed to hazardous chemical (Formalin) from fume hood not operating properly resulting in potential chronic and irreversible illnesses.
