29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about May 7, 2024, the employer Independence Commercial Construction, Inc at the jobsite located at 1210 East Alexis Rd, Toledo, OH 43612, did not ensure employees were protected from falls while performing roof sheathing activities atop a low sloped commercial wood framed structure. Employees were exposed to falls of 16 feet and 9 inches on the roof to the ground below when accessing plywood sheathing from the hoist area, while carrying it along the roof and while nailing it to the wood trusses.
