29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about May 8, 2024, employees were exposed to crushing/pinch point hazards while working on and around the AccurPress Press Brake (Model No. 725012). The employer did not ensure the point of operation was properly guarded. (b) On or about May 8, 2024, employees were exposed to amputation hazard and crushing/pinch point hazards while working on and around the AccurShear Shear (Model No. 25010), in-that the point of operation and the shear's hold downs were not properly guarded.

29 CFR 1910.303(b)(7): Mechanical execution of work. Electric equipment shall be installed in a neat and workmanlike manner. (a) On or about May 8, 2024, employees working on and around the AccurShear Shear (Model No. 25010) were exposed to an electrical shock hazard. The energized wires were not properly guarded, in-that the energized wires had the secondary insulation pulled away from the energized wires.
