29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 and 1/2 inches: On or about May 8, 2024, employees working at Pit 2 are exposed to tripping hazards while walking across the grate covering the receiving pit and the employer has not ensured that openings in the grate are maintained at less then 2 and 1/2 inches in width.
