29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. At a temporary worksite located at 1901 University Blvd, North, Jacksonville, FL 32211: On or about May 8, 2024, the employer exposed employees to eye injury hazards, in that eye protection were not utilized by employees who were operating nail guns to nail down wood.  b. At a temporary worksite located at 1901 University Blvd, North, Jacksonville, FL 32211: On or about May 8, 2024, the employer exposed employees to eye injury hazards, in that eye protection were not utilized by employees who were using a torch to apply a modified roof system.    c. At a temporary worksite located at 1901 University Blvd, North, Jacksonville, FL 32211: On or about May 8, 2024, the employer exposed employees to eye injury hazards in that eye protection were not utilized by employees using a DeWalt saw to cut sheets of plywood.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 2:12 pitch roof of a temporary worksite located at 1901 University Blvd, North, Jacksonville, Florida 32211:  On or about May 8, 2024, the employer exposed employees to fall hazards ranging from 10 to 12-feet to the ground, in that fall protection were not utilized by employees while performing roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. At front of home ,at a temporary worksite located at 1901 University Blvd, North, Jacksonville, FL 32211: On or about May 8, 2024, the employer exposed employees to a 10-foot fall hazard, in that the siderails of a Werner extension ladder used by the employees to gain access to the roof did not extend three-foot above the upper landing surface (roof).
