On or about May 7, 2024, at the site: employees were exposed to a 12 foot and 17 foot - 4 inch fall hazard when removing roofing materials from a 5:12 pitched roof, in that, fall protection was not used.

29 CFR 1926.1051(a):Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about May 7, 2024, at the site: employees were exposed to a 12 foot and 17 foot - 4 inch fall hazard, in that. the employer allowed employees to access the top of the 5:12 pitched roof from the from lower level by climbing the outside walls of a two-story dwelling, while grabbing the roof line above to pull themselves up to the roof top.
