29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided.  a) On or about May 9, 2024, at Forest Hill Blvd, West Palm Beach FL 33405, the employer did not ensure use of personal protective equipment including, but not limited to eye protection, head protection, hand protection, and hearing protection, while employees were working on a roof using a pneumatic nail gun installing roof tar paper (RTP) without the use of high impact safety glasses or goggles marked ANSI Z87.1, helmets, gloves, or ear plugs/ear muffs.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) On or about May 9, 2024, at Forest Hill Blvd, West Palm Beach FL 33405, employees were exposed to an approximate 10-foot fall hazard while engaged in roofing work without being trained to recognize the hazards of falling and the procedures to follow in order to minimize these hazards while working on a roof without means of fall protection.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) On or about May 9, 2024, at Forest Hill Blvd, West Palm Beach FL 33405, the employer did not ensure the employees were protected from falls while using an extension ladder to access the roof that was not extended 3-feet above the landing and was not secured at its top to a rigid support, thereby exposing employees to a 10-foot fall hazard.
