OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and/or a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm in that employees were exposed to crushed by hazards due to the unsafe operation of automotive lifts:  Shop -  Eagle Equipment auto lift, serial number 09070091, was operated unsafely and/or with defects in the following instances:  a) Lift was not inspected. b) Drivers side rear swing arm restraint was missing. The other three swing arm restraints were not properly functioning. c) Swing arm pads were missing on three of the four swing arm attachments. d) A hydraulic leak was seen near the main hydraulic line, from the pump to the lift components. e) Safety warning labels were covered with other safety stickers. f) The overhead travel limit switch was not operational. g) There were no documents available for employees to show where a vehicle should be lifted. h) Telescoping lift arm over travel stops were missing.
