29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA: On or about May 9, 2024, employees were exposed to struck-by hazards at or near 700 Richmond Rd. Richmond Hts. OH, 44143, where the employer did not have the following traffic control signs or devices in use to protect the crew that had two lanes closes of traffic closed for new water line installation purposes on an active temporary construction zone: The employer did not ensure that traffic control devices were properly placed for a merging taper during a single lane closure on a multilane highway, according to Table 6C-4 Formulas for Determining Taper Length and section 6G.12 per the MUTCD.
