Richmond Hts. OH, 44143, where the employer did not have the following traffic control signs or devices in use to protect the crew that had two lanes closes of traffic closed for new water line installation purposes on an active temporary construction zone:  The employer did not ensure that traffic control devices were properly placed for a merging taper during a single lane closure on a multilane highway, according to Table 6C-4 Formulas for Determining Taper Length and section 6G.12 per the MUTCD.
