To include, but not limited to: -Truck safety related topics including workplace related topics; -Surface conditions where the vehicle is to be operated. Certification to include, but not limited to: -The name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. This condition exposed employees to struck by hazards.   Condition noted on or about 5/9/2024.
