29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) Leadec Corp located at 799 Abbe Road, Sheffield Lake, Ohio, 44054: On or about April 26, 2024, the employer failed to ensure that employees were protected from falling through the hatch openings of the wellhouses of the phosphate tank line in the Paint Department. Employees were exposed to falls of approximately 4.5 feet while performing tank cleaning activities.

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Leadec Corp located at 799 Abbe Road, Sheffield Lake, Ohio, 44054: On or about April 26, 2024, employees were exposed to hazards such as, struck by hazards in that the overhead carrier conveyor in the Paint Department was not deenergized prior to employees engaging in tank cleaning activities. The employer failed to ensure that energy control application steps were implemented to control hazardous electrical and mechanical energy sources.

29 CFR 1910.147(f)(2)(i):When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Leadec Corp located at 799 Abbe Road N., Sheffield, Ohio, 44054: On or about April 26, 2024, the employer failed to ensure that an outside contractor was notified of lockout procedures for energy isolation of the overhead carriage conveyor, located in the paint department. As a result, the contracted employees performing tank cleaning operations in this condition were exposed to hazards such as, but not limited to, struck by hazards created by the moving conveyor.
