29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) On or about May 9, 2024, the employer did not ensure that employees unloading Unit Load Devices (ULDs) were not exposed to unprotected sides or edges that are 4 feet or more above a lower level. Employees were  standing on top of boxes approximately 7 feet in height during break down of the ULDs at the worksite.   No abatement certification or documentation is required for this item.

29 CFR  1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a) On or about May 8, 2024, doors designated as emergency exits were locked on the west end of the facility located at 9740/9742 W. Foster Ave., Chicago, IL 60656.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

(c) On and before May 9, 2024, the employer did not ensure that employees that operated the Doonsan BC32S-7, S/N: FBA1D-1360-00144, performed evaluations on a daily basis before each shift at the facility.
