29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval and capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: Facility: On, or about, May 10, 2024, Hyster forklift 23036 was equipped with longer forks than what was listed on the identification plate. Also, markings on the forks extended the load center from 24 inches to 53 inches, which reduced the capacity of the forklift by approximately 40%. The longer forks along with the reduced lifting capacity were not added to the identification plate.
